• all previous marriages (date and place of marriage, spouse name, spouse date of birth)

• dates, and time period of staying out of the USA since you become resident (also the country you visited)

• 2 passport size photos of yourself

• point of contact: address, phone number, email address

• $300 money order filled out for Pataki Center
